Before You Book – What You’ll Need
You’ll need to gather these items before your appointment:
Physician Recommendation Form (OMMA version, signed within last 30 days) – [Adult Patient PDF] | [Minor Patient PDF]
Passport-style photo (head & shoulders, plain background, no hats/glasses) – [Photo Requirements PDF]
Proof of Oklahoma Residency (any ONE):
- OK Driver’s License or State ID
- OK Voter ID card
- Utility bill (last 6 months)
- Residential lease/mortgage
Proof of Identity (any ONE):
- OK Driver’s License or State ID
- U.S. Passport
- Tribal ID accepted by OMMA
Application Fee:
- Standard: $100 + $4.30
- Reduced (Medicaid, Medicare, SoonerCare): $20 + $2.50
- Temporary: $100 + $4.30
- Caregiver: No fee
How It Works
- Complete Forms
- Choose the correct physician form (adult/minor)
- If you’re a caregiver, also complete the Caregiver Designation Form
- Book Your Appointment
- We’ll meet with you in-person or online to review your documents and help with your OMMA portal application.
- Submit & Pay
- We guide you through uploading documents, filling details correctly, and paying the application fee to OMMA.
- Receive Your Card
- OMMA mails your approved license to your address (usually within 14 business days).
